How to notify the Office of your address change?

  • In writing, by completing the form available on our website: address change declaration template and send it to our address.
  • Online, through the form on WSC Contact
  • If you are moving to a different voivodeship, complete this form
If your new place of residence is in a different province, your case will be transferred to the appropriate regional office based on territorial jurisdiction.
