When should I apply?

An application for issue of document confirming the right of permanent residence of an EU citizen should be submitted in person, after 5 years of continuous residence in the territory of the Republic of Poland.

The requirement to appear in person does not apply to minors.

Where should I apply?

You may submit the application in person at the Foreigners' Affairs Department in Poznań or at one of the office's delegations. Check the addresses 

You book a visit to submit your application in advance via the website: https://www.poznan.uw.gov.pl/rejestracja/ 

If you are not able to submit your application in person, please send your application on paper by post.

What documents do I need? 

  • A photocopy of a valid travel document or other document confirming your identity and citizenship (original for inspection). In justified cases, if you do not hold a valid travel document and or other document confirming your identity and you cannot obtain it, you may present another document (other than official documents) confirming your identity and citizenship in an unambiguous manner. 

  • 5 up-to-date photographs  - undamaged, in colour, of good focus and quality. Size: 35mm x 45mm. Taken within the last 6 months. Head should take 70-80% of the photo (from the bottom of the chin to the top of the head). The photo should be taken in front of a plain white or off-white background. Photo should be taken in full-face view directly facing the camera, with eyes open, neutral face expression and mouth closed. The photo should also reflect your natural skin colour. If you are a person with congenital or acquired sight defects, you may enclose photographs of with dark glasses. If you wear religion-related headgear – you may enclose photographs with such headgear. The headgear, however, cannot make it impossible to identify you.

  • documents confirming the 5-year continuous residence in Poland,

  • documents stating the reasons for breaks in stay, if any.

What should I know?

What are my obligations, once the document has been issued?

You should inform the Governor competent for your place of residence in the following cases:

  • the data placed in the document have changed;
  • the document has been damaged;
  • the occurrence of other circumstances which make your identification  difficult or impossible

In the case of loss of the document confirming the right of permanent residence of an EU citizen, a new document is issued.
