Check-list - Issue of a residence card on an application

  • 1 copy of the application, completed as instructed (download the application form). You can also obtain the application form at any of our offices. Check the addresses.
  • 2 photos which should meet the following requirements:
    • Undamaged, in colour, of good focus and quality,
    • size: 35x45mm,
    • not older than last 6 months,
    • head should take 70-80% of the photo (from the bottom of the chin to the top of the head).
    • photo should be taken in front of a plain white or off-white background,
    • photo should be taken in full-face view directly facing the camera, with eyes open, neutral face expression and mouth closed. Hair should be uncovered. The photo should also reflect your natural skin colour. Eyes and pupils should be clearly visible, the line of the eyes should be parallel to the upper edge of the photo.
  • A valid travel document (original for inspection). In justified cases, if you do not hold a valid travel document and cannot obtain it, you may present another document confirming your identity.

  • Confirmation of registered residence/the registered residence (temporary or permanent).

You will need also an original of  the stamp duty payment to start the proceedings.

All documents are submitted in original/certified copy and in certified translation if required.

Dictionary

Original

It is a document that is not a copy. In some cases, you will be allowed to submit a copy of the original document, certified by an officer with a signature and stamp when submitting the document. A copy can be also authenticated by a notary or by your lawyer or your legal adviser if he/she represents you as attorney-in-fact in your legalization procedure.

Photographs/biometric photo

It is an undamaged photo, coloured, with a good focus; with dimensions 35 mm x 45 mm; taken not earlier than within 6 months before the date of submitting your application; showing your face image from the top of head to upper part of your shoulders, in a way of 70-80 % your face is on the photo, on a bright uniform background, showing you looking straight with opened eyes, uncovered hair, with a natural face expression and closed mouth, and also imaging a natural skin colour; showing clearly your eyes, especially the pupils, your eye line is parallel to an upper edge of photo.

Residence card

It is a document that you are issued with after granting you your residence permit.

A residence card, during its validity, confirms your identity throughout your stay in Poland.

Together with a valid travel document (passport), it entitles you to multiple crossing the Polish border.

You will be issued your residence card after you pay the stamp duty of 50PLN (25PLN in a case of a minor and a student), and submission of registered residence document, or a statement about residence card without an address stated on it (download it here)

Temporary residence and work permit/ Fixed term residence and work entitlement

A fixed term residence and work entitlement is a temporary residence permit granted when your purpose of stay in Poland is to work. This permit entitles you to stay in Poland and work for the employer under the conditions specified in the decision. More information 

Valid travel document/passport

It is a document which entitles you to the border crossing.
