Check-list - Short-term stay
Documents required to obtain a stamp in your passport (i.e. formal requirements)
- Application form - fill in the on-line application or fill in the application in accordance with the instructions in 1 copy (application to be downloaded).
- 2 photographs,
- A photocopy of a valid travel document (the original for inspection), in a particularly justified case, when you do not have any valid travel document and you are not able to obtain one, you may present another document confirming your identity.

Confirmation of the payment of stamp duty is required to begin the proceedings.
- if you are obliged to appear in person in the Polish public authority or the country’s interest requires your presence in Poland - your stamp duty is 85 PLN
- if an unique personal situation requires your presence in Poland - your stamp duty is 340 PLN
Documents needed to grant a permit:
- Documents confirming the circumstances requiring your stay in Poland (e.g. request to appear in Polish public authorities, medical certificates, etc.)
In the case of need to clarify or clarify the evidence held by the authority during the proceedings, you may be requested to provide other documents or to confirm the circumstances referred to in the application.All documents are submitted in original / certified copy
