Photographs must be undamaged, in color, and of good quality; sized 35 mm x 45 mm; taken no earlier than 6 months before the date of submitting the application; showing the foreign national’s face from the top of the head to the upper shoulders, so that the face occupies 70–80% of the photograph, against a uniform light background, with the person looking straight ahead, eyes open and visible, hair not covering the face, a natural facial expression, and closed mouth, while also accurately reflecting the natural skin tone. The photograph must clearly show the foreign national’s eyes, especially the pupils, and the eye line must be parallel to the upper edge of the photograph.

For applications for temporary residence permits referred to in Article 139a(1) and Article 139o(1), as well as permits referred to in Article 159(1) and Article 160 points 1, 3, 4, or 6 of the Act, submitted in paper form and granted to a foreign national staying outside the territory of the Republic of Poland, four photographs of the foreign national must be attached.

A foreign national who wears head covering in accordance with the rules of their religion may attach to the application a photograph showing them wearing such head covering, provided that the face is fully visible. In such a case, a statement by the foreign national confirming membership in a religious community must also be attached to the application (statement template available for download).
