Important Information When Applying for a Temporary Residence Permit for a Child Under 6 Years Old

When applying for a temporary residence permit for a minor under the age of 6, you must attach a photocopy of the child’s valid passport (the original must be shown in person). This requirement is the same as for all other temporary residence applications.

Important Notice: We do not accept notarized or certified copies. The original passport must be shown either at the main office in Poznań or at branch information points. Please bring both the original passport and its photocopy.

If the child does not have a valid travel document and you cannot obtain one, you may submit another document confirming the child’s identity. Children under 6 are not required to appear in person when submitting the application, and fingerprints are not collected.

You can present the original passport at at the service point of the office in Poznań or at the information desks in branch offices.

Application Requirements

When completing the application for a child under 6, please ensure the following:

  • Use Polish. If you do not speak the language, seek help from a translator.

  • Fill out all required fields truthfully. Missing or false information may negatively affect the outcome.

  • Inform us of any previous personal data you provided in the past. If the child has changed their name, include previous names in the explanation.

  • Provide the child’s actual place of residence. If the parent/guardian's mailing address differs from the child’s residence, notify us at the time of application.

  • Do not withhold information. You submit the application under penalty of law.

  • Sign the application yourself as the child’s parent or court-appointed legal guardian. Use Latin script and include a supporting document (e.g. child’s birth certificate with sworn Polish translation or a court decision on guardianship).

  • Include your contact details — phone number and email — to make communication easier.

Reminder: By signing the application, you legally declare that all information is true. Submitting false information or fake documents can lead to rejection of the application.

Do Not Send the Original Passport by Mail! You may present the original passport of a minor child under 6 years of age at the service point of the office in Poznań or at the information desks in branch offices.
