Check-list - Stay with a minor child who is a Polish citizen

• Online application completed in the Case Handling Module,

• Scans/photos of all pages of a valid travel document (original available for inspection). In particularly justified cases, if you do not possess a valid travel document and are unable to obtain one, you may scan another document confirming your identity,

• 1 digital photograph,

• confirmation of the payment of stamp duty. 

Documents required to be granted the permit:

• copy of Polish ID or passport of your minor child, if they have one, or any other document confirming their Polish citizenship,

• child's birth certificate.

• health insurance,

• source of stable and regular income.

  All documents are submitted in original / certified copy.
