Check-list - Registration of the stay of an EU citizen's family member who is an EU citizen
- Application completed in accordance with the instructions in 1 copy (application to be downloaded). You can receive a copy of your application form in our offices,
- Photocopy of a valid travel document or other document confirming your identity and citizenship (original for inspection). In justified cases, if you do not hold a valid travel document and or other document confirming your identity and you cannot obtain it, you may present another document (other than official documents) confirming your identity and citizenship in an unambiguous manner.
- The certificate of registration of residence of an EU citizen, issued to an UE citizen, with whom you reside on the territory of Poland;
- Other required documents:
- In the case of a spouse of an EU citizen – a document confirming contracting a marriage;
- Ascendants of an EU citizen (direct ascendant of an EU citizen or his/her spouse, a dependent of an EU citizen or his/her spouse) - a document confirming the existence of family ties, and proof of age or certification of dependence on the European Union citizen or his/her spouse;
- Descendants of an EU citizen (direct descendant of an EU citizen or his/her spouse, to the age of 21 years or a dependent of an EU citizen or his/her spouse) - a document confirming the existence of family ties and proof of age or certification of dependence on the European Union citizen or his/her spouse.
