NOTE! This form should be submitted only in the case of the amendment of the already granted residence and work permit and the procedure reffered to here.
This form is appropriate only for applications for a amendment of a temporary residence and work permit submitted before 29th January 2022 and to applications for amendment of a temporary residence permit regarding work in a profession requiring higher qualifications (EU Blue Card). If you want to submit an application for amendment of the temporary residence and work permit after 28th January 2022. go to this form.
