This form is appropriate only for applications for a amendment of a temporary residence and work permit submitted before 29th January 2022 and to applications for amendment of a temporary residence permit regarding work in a profession requiring higher qualifications (EU Blue Card). This is the form of employer declaration on entrusting work for persons who have received a residence permit under the Act of 17 December 2021 amending the Act on foreigners and other acts (Journal of Laws of 2022, 91) so called conditional permit - all information regarding submittng the declaration can be found in the instruction of the decision. The declaration should be filled in by the employer. There is time limit of 60 days for submitting the declaration, from the date of receiving the decision.
