If you go abroad and you do not appoint an attorney to handle the case, you should at least appoint an attorney for notification in Poland. If you did not indicate an attorney for notification, the letters intended for you will be left in the case file with the result of being delivered.

If you have appointed a proxy, attach the original power of attorney to the documents. If you have more than one attorney, remember to indicate which you want us to send the correspondence to you.
Originals/translations
Submit documents in original (for inspection) or authenticated copy. If you send the documents by post, send them in the original along with a request for their return, after it will be used for the official purposes - after the proceedings have been completed, we will send the originals back to you.
If you submit documents in a foreign language in the application, it is also required to submit a translation of the document into Polish by a certified/sworn translator. You can find a list of certified translators on the Ministry of Justice website.
Loss/damage of the document
In the case of loss/damage of the residence card, you should notify the authority who issued your card, within 3 days from when the loss/damage of the document has occurred. The notification should be submitted using this form.
Once you submit the notification about the loss/damage of your document, we will issue a free of charge certificate that confirms the loss/damage of the document. This certificate is valid until a replacement document is issued (but not longer than 2 months from the date of issue).
If you recover your lost document in the meantime, you should notify the authority who issued the document within 3 days from the date of recovery. If you have already received a replacement document, you should promptly return your recovered document to the authority, which has issued it.
Return of the document:
You should return the Polish travel document to the Governor, who issued it originally also if:
- you have been granted Polish citizenship, or
- you have received a decision revoking your Polish document, or
- you have received a decision revoking your permanent residence permit, or residence permit for long-term EU resident, or residence permit for humanitarian reasons, or
- you have received a decision to deprive you of subsidiary protection, or
- you have received a valid travel document.
You should return your Polish travel document as soon as possible, but no later than within 14 days from the day on which:
- you have received a document granting you the Polish citizenship, or
- one of the above-mentioned decisions became final.
At your request, we may issue a free certificate of return of the Polish travel document. The certificate is valid for 30 days.
