ISSUE/REPLACEMENT OF THE RESIDENCE CARD

A fee of PLN 50 should be paid for printing the card and a valid confirmation of registered address should be provided to the Office if you would like your address details to be printed on the card; or a request for a residence card to be printed without a registered address on it.

  • We issue a residence card after granting a temporary residence permit, permanent residence permit, long-term resident's EU residence permit, residence permit for humanitarian reasons;
  • In order to obtain a residence card, you must apply for one of the above mentioned residence permits. Before submitting the application, make sure you are entitled to the permit requested.

The Consul issues visas.  Province Governor has no impact on Consul’s decision regarding visas.

No. The invitation will be sent to the address you have indicated in the application form.             But if you prefer to collect the invitation in person from our Office, you should inform about it during the procedure.

Yes. You can apply for visa extension or for the temporary residence permit/permanent residence permit.

You should send the invitation (in original) to the foreigner you are inviting. S/he should submit the received invitation together with the visa application in the consulate when applying for visa.

A hotel booking confirmation should contain information on the person booking a room, the person for whom it is booked and the period.

In this case together with the application for invitation you should submit written consent of the owner of the flat, in which s/he agrees to provide accommodation for the foreigner together with property deed.

Yes, current cadastre is sufficient if it contains description of the property together with the address. It also need to indicate the owner of the property (electronic version of the cadastre will be sufficient).

Yes, but you are oblige to show original of the document only for inspection. Document can be also authenticated by a notary. Your lawyer or your legal adviser can also authenticate a copy of the document if he/she represents you as attorney-in-fact in the invitation procedure.
