When should I apply?
You apply for the the replacement of a new document certifying the permanent residence right within 14 days after the occurrence of the premises for the exchange of the document if you have lost your current document, or any details included in your certificate have changed, i.e. you have changed your name, or your document has been damaged or your face has changed significantly. If validity period of your document expired - apply for a new certificate within at least 30 days before the expiry of the validity of your current document.
You submit your application in-person.
In very exceptional circumstances, including your health condition, it will be possible to submit your application not only in person.
The requirement to submit an application in person does not apply to minors.

Where should I apply?
You can send your application by post to the Department for Foreigners in Poznań or one of the office's branches.
The date of application submission is determined by the date of posting at the Post Office.
The next step will be an in-person visit to the office (if required). During the visit, you will also complete any additional required documents.

What documents do I need?
- Application completed in accordance with the instructions in 1 copy (application to be downloaded). You can receive a copy of your application form in our offices,
- photocopy of a valid travel document or other document confirming your identity and citizenship (original available for inspection), if you cannot present a travel document or another document confirming your identity and citizenship, you can provide other evidence than an official document that will confirm your identity and citizenship in no doubt way,
- 2 photos,
- your current document confirming the right of permanent residence in the case of replacement.
