If you are a citizen of Ukraine, you reside in Wielkopolska Provice and you work as a driver in international transport and you stay in Poland on the basis of a national visa, which was extended under the Act of 12 March 2022 on assistance to citizens of Ukraine in connection with an armed conflict on the territory of this country, you can get a visa sticker to confirm it. This solution can only be used by those citizens of Ukraine - drivers in international transport, who are holding a national visa with the symbol "D", which:

entitles them to stay in Poland for the period ending on24th February 2022 at the earliest, or ending in the near future, but no later than 24th August 2023

was issued for the purpose of work: symbol 05a or 06

was issued for a period shorter than 1 year (less than 365 days of stay)
You do not need to submit any application to receive a visa sticker.
Just send u san email to the following address: naklejka.wiza@poznan.uw.gov.pl, providing your residential address in Wielkopolska Province, as well as a contact phone number and attach:
- a passport scan (page with personal details and visa page),
- a scan of the driver's certificate or current medical and psychological tests, driving license and professional qualification certificate
- a scan of the employment contract or other contract confirming the basis for the performance of work, which will result in the type of work consisting in the transport of people or goods across the border, and if the contract does not contain such content, other documents, e.g. scope of activities, employer's certificate, from which the kind of the work performed will be visible.

To pick up a visa sticker::

you must have a valid travel document (passport) with you,

We do not charge any fees for the visa sticker.
