When should I apply?

You should apply for issue of the document confirming the right of permanent residence of an EU citizen after 5 years of continuous residence in Poland.

You should submit your application in-person.

The requirement to appear in person does not apply to minors

Where should I apply?

You may apply in person at the Department for Foreigners in Poznań or one of the office's delegations: Book your visit

If you cannot apply in person , please send your application by post.

What documents do I need? 

  • Your filled in application form in 1 copy (download application form),

  • A photocopy of your valid travel document or other document confirming your identity and citizenship (original for inspection).
    In justified cases, if you do not hold any valid travel document and or other document confirming your identity and you cannot obtain it, you may present another document (other than official documents) confirming your identity and citizenship in an unambiguous manner. 

  • 5 up-to-date photographs,

  • documents confirming your 5-year continuous residence in Poland,

  • documents stating the reasons for breaks in stay, if any.

What should I know?

What are my obligations, once the document has been issued?

It would be best if you informed the Governor competent for your place of residence in the following cases:

  • the data placed in your document have changed;
  • your document is damaged;
  • any occurrence of other circumstances which make your identification difficult or impossible.

In the case of loss of the document confirming the right of permanent residence of an EU citizen, we will issue a new document.
